Anton Danyluk and Elma Pazar also gasp in shock as the host reveals that one person from each couple is about to be booted from the villa.
Last night, the Islanders were forced to choose which pairing they believed was the least compatible in the ITV2 dating show, with the foursome getting the lowest amount of votes.
They then went head to head as Love Island fans voted for which member of each couple they wanted to save.
In scenes that will air in tonight's episode, the four hopefuls look on in horror as Caroline explains the bombshell twist to them.
The 39-year-old star says: "The public have been voting to save one Islander from each couple.
"Either Elma or Anton and Lucie or Joe will be dumped tonight."
All of the Islanders react in horror, with Tommy Fury, Molly-Mae Hague, Amber Gill, and Michael Griffiths also appearing concerned as they await their castmates' fate - with the two that do stay on becoming singletons in the villa.
Fans at home think that Joe and Elma's time could be up, with one viewer tweeting: "If Joe and Elma aren't the ones going home there's going to be absolute scenes."
A second agreed: "I bet you Elma and Joe are going home tonight."
Joe's behaviour has come under fire in recent episodes, with the 23-year-old caterer's controlling attitude towards Lucie sparking a backlash from fans.

Since the show launched earlier this month, Joe has appeared possessive of Lucie - with fans even comparing him to the psychotic stalker Joe in Netflix series You just two days after they first coupled up.
Things only got worse at the weekend, with Joe growing jealous of Lucie's close relationship with hunky boxer Tommy - demanding she "changed" and spent time with the other girls instead.
Meanwhile, Elma only joined the villa last week - entering alongside fellow newcomer Maura Higgins.
The Essex girl managed to steal Anton from Anna Vakili, who he'd promised to maintain a platonic coupling with, and later caused shockwaves when she admitted that she was keeping her options open and had her eye on new boy Tom Walker.
Viewers praised Elma for putting jealous love rival Maura in her place in last night's episode, with the Irish beauty admitting that she was "f***ing raging" at Elma's refusal to rule out Tom as a partner.
