Alec Baldwin's stunt double reportedly accidentally fired two rounds of ammunition prior to the incident.
That double misfire was one of many safety concerns raised by crew members on the film's New Mexico set.
Hours before the shooting, a number of people employed to work on the film walked off over safety fears.
WHAT WAS ALEC DOING BEFORE THE SHOOTING?
Just hours before the violent mishap, Alec, 63, posed in character on the set and posted to Instagram.
Dressed in old Western gear, including dirty cowboy boots, a vest and bandana, he was seen holding a hat looking tired in front of his trailer.
“Back to in person at the office. Blimey…it’s exhausting,” he captioned the photo.
The post has since been taken down.
FILMING PAUSED
Due to Thursday’s tragic events, filming has been paused.
A rep said: “Production has been halted for the time being on Rust. The safety of our cast and crew remains our top priority.”

HALYNA'S FATHER DOESN'T BLAME ALEC FOR HER DEATH
The distraught father of shot cinematographer Halyna Hutchins yesterday insisted Alec Baldwin was not responsible for her shocking death.
Speaking for the first time since the tragedy, Anatoly Androsovych said the blame lay instead with the movie armoury team who handed the actor a loaded revolver.
Ukrainian Anatoly told The Sun on Sunday last night: “We still can’t believe Halyna is dead and her mother is going out of her mind with grief.
"But I don’t hold Alec Baldwin responsible — it is the responsibility of the props people who handle the guns.”
FILM ARMORER SHARES THREAD OF INFO ON TWITTER
S.L. Huang, a Hugo-award-winning author and film armorer, spent time on Twitter answering commonly asked questions about guns on film sets.
"Blank guns are real guns. Semi-autos are what we call "blank adapted" but that is purely for the *function* of the gun, not for safety (a projectile could still exit)," Huang wrote.
The vintage Colt pistol shot on set could have discharged a bullet, or a piece of shrapnel that was lodged in the chamber.
THREE PEOPLE HANDLED THE GUN FOR THE SCENE
Rust director Joel Souza said three people handled the gun that shot a live round, which killed Halyna Hutchins.
The affidavit said it was first handled by armorer Hannah Gutierrez Reed, then assistant director Dave Halls, and then Alec Baldwin.

The armorer was described as being “inexperienced and green” by a Rust production source.

They insisted that she should have checked the weapons were safe before handing it to Baldwin.
WHAT HAS ALEC BALDWIN SAID OF THE SHOOTING?
Alec Baldwin tweeted, “There are no words to convey my shock and sadness regarding the tragic accident that took the life of Halyna Hutchins, a wife, mother and deeply admired colleague of ours.”

“My heart is broken for her husband, their son, and all who knew and loved Halyna,” he concluded.

One of Halyna’s college friends Yana Nestoliy said: “She was a wonderful woman and a dear friend and mother.
“Her parents are devastated and are trying to get to the US to see her but it is very difficult for them. I heard from another friend that it was Matthew who called them to break the news.
“She was fantastic at her work and after she left Kiev University she moved to the United States but she would often come back to Europe to work.
“Her mum wants to go to the US to see her and find out what exactly happened but is having trouble with her documents.
