Irish model has made the brave decision to finally sort his head and lifestyle out
JEREMY McConnell was seen leaving his Dublin home to head to rehab earlier today.
The 27-year-old has gone in to treatment to battle his booze and drug demons - as well as finally deal with the grief of losing his mother and siblings in a short space of time at a young age.
He has also been struggling to cope with the sudden loss of his father last month after a short battle with cancer.
She wrote: "Today is the day. I'm so proud of you for finally doing this. Do it for me and Caben but most importantly Do it for your self.
"I know you got this now.Say goodbye to your demons &start a new lease of life with happiness.I have every faith in you."
Jeremy will spend the next few weeks un-contactable at a £400-a-night rehab facility in the heart of the Irish countryside.
While he won't see her or Caben for several weeks, he will have an odd reminder of her with him.

Jeremy was seen pulling along the white plastic suitcase given to him on the January 2016 series of Celebrity Big Brother, where he met Steph and fell in love with her in front of the nation.
His admission comes days after she told the world that her baby's father boyfriend would be undergoing treatment for several weeks, telling OK.co.uk that she had banned him from seeing their infant son until he was better.
In an emotional interview she said: "From the beginning of last year, from when I was trying to prove to everyone I was pregnant and everything like that, Jeremy has had a drug problem.
"He has agreed to go to rehab for six weeks. It will be quite immediate.
"Jeremy knows that he is 100% in the wrong. And he has done things that aren’t good. But he isn’t a bad person, deep down, he’s not an evil vindictive person.
"He has had a drug problem which he has now admitted to, which is brilliant because for the whole year and the half, when someone can’t admit they have a problem – that’s the problem.
"And now he has admitted it. And so I’m willing to stand by him and help him.
"I’m not an idiot and I’m not a mug, and he knows he’s been absolutely appalling and I’ve said that to him. He knows how absolutely lucky he is to have another chance with me.
"He knows how bad he’s been. But I do love him. I never stopped loving him."
He wrote: “I want to leave this last message here for people to maybe get a better understanding of who I am as a person.
“I know a lot of people are sick of reading stories etc in the press about me and my life and have a huge dislike for me, for the record nobody dislikes myself more than me.
“I never once have used my misfortunes in life for sympathy or excuses, as a lot of people have really tough lives, the fact I acted in a way that has been irresponsible a lot of which I feel was self reflection.
“I’ve let my family, friends, management and work down, I just hope I can wipe the slate clean and come back with a second chance.
“Someone in this world when you’re low and you really don’t feel you have anyone and the press and peoples’ opinions on you are not really who you are, it’s tough to try and survive.
“I punished myself more than anyone could, I always said I never cared what people thought of me but deep down it’s been that supportive comment on Instagram or Twitter that’s made me feel a lot better.
“The people who know me know I’m a good guy with a massive heart, I just never handled fame, money or actions very well and got lost in a downward spiral, it’s something I’m about to fix and never look back on.
"But I’ve learnt from my mistakes, this will be my last message for a while.
"I’ve decided to take the appropriate action for myself and my new family and thank you for everyone’s support and to a new lease of life."
