SIMON Cowell is one of the most famous faces in the world thanks to his global talent show empire.
The extent his appearance has changed over the decades has led some fans to wonder whether the media mogul has had plastic surgery.
Has Simon Cowell had plastic surgery and what procedures has he had?
Facelift
Simon Cowell has been very honest when it comes to the cosmetic surgery he has had.
The Britain's Got Talent star has previously admitted that he underwent a treatment in 2018 dubbed the 21st century facelift.
The TV mogul visited cosmetic surgeon Dr Jean-Louis Sebagh, known as the “Picasso of dermatology”, for a Silhouette Soft Lift.
Costing around £2,000, the procedure involves sewing bioplastic-infused thread into the face and neck, before tugging it to get rid of sagging skin.

Speaking to The Sun on Sunday, Simon said: “There’s lots of things you can do now.
"You don’t just have to stuff your face with filler and Botox.
“It hurts like hell but it gets rid of sun damage and unplugs all the crap.
"For me now, it’s all about having clean skin.

"If you have clean skin, you look better.
"But you have to be very careful with some of this stuff.”
Botox and filler
Simon was also one of the first male TV stars to admit to having Botox and filler.
He told The Sun: "There was a phase where everyone was having their faces pumped full of this and the other.
“I saw a picture of me from ‘before’ the other day and didn't recognise it as me first of all.”
However, in April 2022 the TV star decided to stop using injectables when his son Eric was “in hysterics” at his face.
“Enough was enough," Simon insisted.
"There is no filler in my face at all now. Zero."
In May 2024, double-board certified facial plastic surgeon Dr Konstantin Vasyukevich told The U.S. Sun he sees signs of filler, an eyelid lift, and possible "pillow face."
Back in 1994, Simon was still years away from being a household name but was already working in music.
A photo of the then-35-year-old on the red carpet at the Brit Awards shows his "natural features," according to Dr Vasyukevich.
He said the music exec had a "very good jawline and cheeks that weren't particularly full".
Analysing photos of Simon's face from 2019, Dr Vasyukevich said: "Maybe it’s a natural result due to weight gain, but his cheeks appeared higher and the overall face shape and features are slightly pronounced.
"You can see he lost volume in the face, and the overall decreasing roundness and definition of the face.
"However, the cheeks were still very prominent and high."
He added that "something seemed incongruent," which could have been a sign of filler, but "fillers, by themselves, are neither good nor bad".
Dr Vasyukevich noted that Simon's brow had lowered and was closer to his eyes, which could have been a natural effect of aging or Botox to relax the muscle.
"He had a skinfold on the upper eyelid, but since the brow lowered, the skinfold disappeared, so there might have been a blepharoplasty," the doctor explained.
A blepharoplasty is an eyelid lift.
Dr Vasyukevich observed that over the course of two years from 2019 to 2021, the overall volume of the AGT judge's face increased.
"At this point, there could be filler in multiple areas," he admitted.
"Moreover, the forehead and cheeks presented a bumpiness that could be the result of filler.
Between 2022 and 2023, he added that there was an overall "uneven appearance and bumps," which he speculated were due to old filler and migration.
Signs of 'bad work'
Dr Konstantin Vasyukevich shared telltale signs of bad cosmetic work.
- Unnatural appearance: "This is the opposite of what we strive for," he said.
- Loss of facial expression: "A skilled surgeon understands facial anatomy and avoids procedures that restrict natural movement," he explained. "If someone struggles to smile or frown freely, it might indicate the underlying muscles or tissues were compromised during surgery."
- Asymmetry: "The human face is naturally asymmetrical, but a skilled surgeon aims for a balanced result," he said. "Obvious asymmetry, like uneven brows, a crooked nose, or mismatched lips, can be a sign of poor technique or inexperience."

Simon Cowell was born in London on October 7, 1959.
He left school at 16 and got a job at the record label EMI, working in the mailroom.
After working his way up the career ladder, he is now one of the biggest media moguls in the world.
In an interview with The Sun about his impressive life, the BGT chief still feels younger than ever.
Speaking to us, Simon said: "When I was 40, I thought 'what am I going to do when I'm 60', but to be honest with you I feel the same as I did then."
He also opened up about his plans for retirement — and it appears he's already got it all figured out.
The TV star revealed: ''I'll never retire. I would never, ever do that. I think you've got to have a job you hate to retire. Thank God I have a job I still love.''
