Khoe's sister, Kim, turned 43 on October 21.
In the nearly blurry snapshot, The Kardashians star donned a face mask resembling the birthday girl.
Kim appeared to be smiling on the mask and showing her plump lips.
With the mask still on, Khloe then covered her eyes with reflective sunglasses.
The Hulu star captioned the lengthy post: "Seriously though, I feel so blessed to have you as my sister and best friend.
"You’ve taught me that Nothing can break us unless we allow it to, but everything can strengthen us if we are willing to take on that battle.
She continued: "You have gracefully fought many battles, and you have become stronger because of them."
Khloe concluded: "You inspire me daily. You motivate me to be better."
THE COMMENTS
In the comments section, fans did not understand Khloe's wild sense of humor as she mocked and celebrated her sister.

Khloe responded to the comments and explained the joke: "The first photo is of me wearing a mask of my sister's face.
"I thought it was a joke, but clearly it went over everyone’s head.
"Speaking of heads, everyone should be very concerned that her head was that big."
THE BIRTHDAY PARTY
On Friday night, Kim celebrated her birthday with a star-studded bash.
The Skims founder partied with sisters Khloe and Kendall Jenner, 27, momager Kris Jenner, 67, gal pal Hailey Bieber, 36, and other famous family and friends.
Kim arrived at the Funke restaurant in Beverly Hills, California.
She stunned in a brightly-colored dress that flaunted her fit figure.
Kim strutted into the venue wearing a red sleeveless gown with straps across the chest and cutouts down the side of her booty.
