The online forms require you to upload a photograph of yourself, and you can also submit a video if they want to show off more of your personality.
As well as providing details about yourself, the form also asks you to describe your perfect partner in the hope that the casting team will be able to pair you with a perfect match to the best of their ability.

The First Dates Hotel is based in Italy, an hour from Naples and the Amalfi Coast.
The hotel's real name is and is located within 60 acres of olive groves.
The hotel boats 42 bedrooms, a beautiful outdoor pool and an on-site Michelin-starred restaurant.
Applicants do NOT get paid to take part in First Dates Hotel.
