The train driver appeared to have suffered a brain haemorrhage which led to a devastating crash.

The impact of the collision caused the train to become derailed and then smash into an oncoming vehicle.

With three main characters, Sister Julienne (Jenny Agutter), Nurse Nancy Corrigan (Megan Cusack) and Dr Turner, on board, fans feared the worst as they lay motionless under the rubble after the collision.

But they managed to escape the wreckage and were pulled to safety when Timothy Turner (Max Macmillan) altered emergency services to their whereabouts.

Dr Turner and Sister Julienne were rushed to hospital with their injuries and Nurse Corrigan stumbled out of the carriage with a bloody gash to her head and desperately called out for her colleagues but with no success was sent straight to Nonnatus House.

Sadly not everyone made it out alive, with Lionel Corbett (Marc Elliott) and Dorothy Carnie (Stephanie Jacob) tragically dying in the chaos.

In a heartbreaking turn of events, a mother who gave birth in the episode was revealed as the wife of the train driver, who suffered from a seizure whilst driving the train.

It was later revealed to his wife Edina (Maya Saroya) that her husband was suffering from a brain tumour and was not a reckless driver.

BBC viewers raced to social media to share their devastation at the emotional episode.

One wrote: "#CallTheMidwife always leaves me in floods of tears...tonight's episode was emotional."

Another posted: "Can’t actually cope with the stress of #CallTheMidwife tonight. 😭😭."

A third shared: "Far too stressful for a Sunday evening 😨 #CallTheMidwife."

"I can’t handle this #CallTheMidwife," another teary-eyed viewer added.

Actor Stephen McGann shared a heartfelt post revealing that the episode had "personal context" for him.

He wrote: "Some personal context to tonight's #CallTheMidwife ep. I've been in a deadly disaster myself. It's cruel, but ordinary in its cruel lottery. Absolutely part of life. Also, my mum lost twins like Carol. My brothers. They were placed in a coffin with a stranger. xx"

Season 12 and 13 have been given the green light by the BBC meaning fans will reunite with the beloved midwives of Poplar until 2024.

BBC One confirmed back in 2021 the hit drama will be
