With the score locked at 1-1, the 22-year-old was given the simple task of booting the ball up the field following a back pass from the defender.
But just as he goes to take an almighty swipe with his left peg, the ball rolls over a plastic cup and over his foot.
The keeper’s momentum then puts him to the floor as he watches the ball harmlessly go over the line for a needless corner.
In an incident that resembles the famous beach ball drama in 2009, Real Madrid couldn’t capitalise from the following set-piece.
Bizarrely, Onana then opted to leave cup on the field before trudging back to his goal line.
With the video circulating online, viewers were left confused why he just didn’t clear the litter.
One said: “Doesn’t think to move it afterwards.”
Another added: “Just leaves it there so it can happen again.”
Despite Ajax’s impressive performance, they fell to defeat thanks to Marco Asensio’s late winner.
Karim Benzema opened the scoring with an emphatic finish on the hour mark before Hakim Ziyech thought he had snatched an equaliser.
The return leg will take place in three weeks time at the Bernabeu.
