PARSONS Green Tube bomber Ahmed Hassan was referred to the Government’s anti-terror programme three times but its deradicalisation team failed to see him.
Hassan, 18, was never assigned a mentor despite spending 20 months on Prevent’s radar.
Instead he was given mental health and educational support.
Hassan, of Sunbury, Surrey, was on Prevent’s books when he launched his attack in September, injuring 51 people.
