The healthcare worker, who has been working around the clock in the fight against coronavirus, was left "shaken up" by the incident in the Scunthorpe store.
A warning has been issued to workers by a trust director, telling staff to keep their ID badges out of sight when they are off NHS premises.
It is unclear whether the violation happened during designated NHS worker shopping hours.
Jug Johal, security management director at the Northern Lincolnshire and Goole NHS Foundation Trust said: "Unfortunately, I can confirm that a member of our staff was attacked in a supermarket this morning with the intention of getting her ID badge.
The last thing they need when they are out doing their shopping is having to deal with things like this.
"I'm pleased to report she is shook up but otherwise okay.
"We have offered our support and will be making sure she has everything she needs.
"We have reminded all our staff to be careful when out and about and to keep their ID badges out of sight.
"Our staff at the trust, and all staff across the NHS and in other key sectors, are working day and night to keep people safe and fight this pandemic.
"The last thing they need when they are out doing their shopping is having to deal with things like this. Whoever did this should be ashamed.

The NHS staff member was not injured in the attack.
Yesterday, it was announced that another two NHS nurses died from coronavirus.
Aimee O'Rourke, 38, and Areema Nasreen, 36, sadly passed away after the pair were both treating patients.
Ms Nasreen, a mum-of-three who had no underlying health issues, died this morning where she worked at Walsall Manor Hospital in the West Midlands - after her sister urged everyone to "take this seriously".
Her death came just hours after fellow nurse and mum-of-three Ms O'Rourke, 38, died at the Queen Elizabeth The Queen Mother Hospital in Margate, Kent, where she worked.
A third nurse is today being treated in intensive care at Southend Hospital in Essex. The man, in his 60s, is on a ventilator.
