Severely ill baby Hawk, passed away not long after the photo was taken.

Captured around 11 days after they were born, in this emotional picture Hawk is cuddled by his tiny twin brother Mason.

Hawk Buchmeyer was born with a rare condition Congenital Diaphragmatic Hernia (CDH) in a hospital in Florida, USA.

A serious condition, his parents, Brandy Guettler and Tommy Buchmeyer, knew that his life was at risk and urged friends and family to pray for their baby.

Sadly on Wednesday, little Hawk lost his battle as his family updated their supporters with an emotional message on Facebook.

They announced:  “Our hearts are saddened this morning as our sweet little man was called home to be with Jesus.

“He went very peacefully and we know he’s no longer suffering.

“Please give us some time to grieve and try to start the healing process.”
